Quality of Life in Turkish Ankylosing Spondylitis Patients Introduction Ankylosing spondylitis (AS) is a chronic inflammatory rheumatological disease characterized by spinal inflammation, usually in the form of sacroiliitis and spondylitis which may lead to syndesmophyte formation and ankylosis in the further course of the disease [1]. AS most commonly begins in the second and third decade of life as persistent inflammatory back pain that can already be associated with significant loss of function, work disability and impaired quality of life early in the disease [2-4]. In studies conducted previously, risk factors affecting the functional status and quality of life in patients with AS have been evaluated. Age, duration of symptoms, pain severity, stiffness, peripheral arthritis, total hip arthroplasty, smoking habit of patient, having history of more physically demanding jobs and lower levels of education have been demonstrated as risk factors for functional limitations and quality of life in patients with AS [3-8]. In other rheumatic diseases, level of education has been found to be possibly associated as a risk factor [9-11]. In this study, we planned to determine the possible risk factors affecting functional situation and quality of life in Turkish patients with AS. Materials and Methods Eigthy nine Turkish AS patients diagnosed with AS according to the Modified 1984 New York Criteria who were admitted to the Rheumatology Outpatient Clinics of Akdeniz University and Adnan Menderes University were included in the study. This study was approved by the Research Eth ­ics Committee Yet, one patient, who did not want to participate, was excluded from the study. Thus, a total of 88 volunteer Turkish AS patients consisting of 24 females (27.3%) and 64 males (72.7%) with their age ranging from 21 to 81 were enrolled in the study. Demographic information of the patients was obtained, their heights and weights were measured and these values were recorded. Then body mass indices (BMIs) of the patients were calculated by dividing the body weight as kilograms by the square of height in meters. Levels of education of the patients were investigated and recorded (0: illiterate, 1: literate, 2: primary school graduate, 3: secondary school graduate, 4: high school graduate, 5: university graduate). Dates of diagnosis of the patients, drug(s) they used, their duration of morning stiffness (as minutes) were investigated and recorded. Patients with diagnosis of AS who were admitted to the Rheumatology Outpatient Clinics are routinely instructed with a home-based exercise program. Exercises recommended are as breathing and posture exercises, and range of motion/stretching exercises for all joints. Patients were inquired about the extent they do these recommended exercises, and they were rated as 0 (not doing the exercises) , 1 (doing irregularly or occasionally), 2 (doing regularly every day), and the results were recorded. Smoking habits of the patients were evaluated. Those who have smoked at least one cigarette a day for a period of longer than 6 months during their lifetime were included in the smoking group. Whether or not the individuals in this group were currently smoking and how many cigarettes a day and for how many years they have smoked were determined. â€Å"Packs/year† term was calculated by multiplying amount of cigarettes (as packs) smoked daily by smoking period (as years). Indices have been developed to measure the activation status, functional status, spinal mobility values and quality of life of patients with ankylosing spondylitis. Bath AS Disease Activity Index (BASDAI), developed for evaluating disease activity, consists of 6 visuel analog scale (VAS) measurements comprising of fatigue, spinal and peripheral joint pain, severity and morning stiffness [12]. Bath AS Functional Index (BASFI), developed for functional evaluation, was determined to have been superior regarding sensitivity to the change to the Dougados Functional Index (DFI) which was developed for the same purpose [13]. Bath AS metrology index (BASMI) was developed by evaluating 20 different clinical assessment methods and selecting 5 among them with the property of the highest validity, reliability, repeatability, and sensitivity to the change [14]. Developed to assess the quality of life of patients with AS, Ankylosing Spondylitis Quality of Life Questionnaire (ASQoL) has been demons trated to be a valid and reliable tool that can be used both in clinical practice and in scientific research [15]. To evaluate the functional status, disease activity, spinal mobility and quality of life of the patients, BASFI, BASDAI, BASMI and ASQoL scales were used respectively. All indices were assessed by the same physician. Turkish versions of BASFI, BASDAI and ASQoL were used. The reliability of the Turkish versions of BASFI, BASDAI, and ASQoL has been confirmed [16-19]. BASMI is a combined index comprising five assessments of spinal mobility in AS patients. The index include assessments of lateral lumbar flexion, tragus-to-wall distance, lumbar flexion, intermalleolar distance and cervical rotation [20]. These measurements have been found to be clinically practical and reliable in reflecting axial status [1]. In our study, measurements of tragus-to-wall distance, modified Schober, cervical rotations (mean of the sum of right and left rotations was taken), lumbar lateral flexion difference (mean of the sum of right and left lateral flexion differences was taken), intermalleolar distance were performed by the same physician on all patients to obtain BASMI score. Apart from these mobility assessments performed, occiput-to-wall distance, chin-to-sternum distance, chest expansion, thoracic Schober, lumbar Schober, hand-to-ground distance and intermalleolar distance were measured. As laboratory values of the patients, erythrocyte sedimentation rate (using Standard Westegren Method) and serum C-reactive protein (CRP) levels were measured. SPSS 14.0 (SPSS Inc., Il., USA) software package was used for data analysis. Results for continuous variables were presented as maximum, minimum and mean  ± standard deviation (SD). Three specific ways in which American expansion shaped the Jacksonian period was through the advancement of technology, by way of slavery, and the Indian Removal Act. Jackson used any political and economic means necessary in order to see American frontier regions expand across the nation. Jackson’s Indian Removal policy had some of the most important consequences and paved the way toward American expansion. In the beginning of the Jacksonian era, colonial Americans’ settlements had not yet extended far beyond the Atlantic seaboard, partly because bad roads and primitive technology limited their ability to expand, and because both hostile Indians and British imperial policy discouraged migration beyond Appalachian Mountains. However, all of this changed after Jackson was in office and American expansion was well underway. One of the ways in which American expansion shaped the Jacksonian period was through technology. The economic and political growth as well as American settlement can only be described in detail as a side effect of the advancement in technology. First the rising dominance of trade, commerce, and growth would not have been possible without the development of the first canals and then the railroads. The Erie Canal was one of the most significant and largest transportation projects of the era. In today’s society public police work hand and hand with private security on a regular basis, either in public or in private functions. Also in today’s society you see off duty cops working as a part-time security officer at a local retailer or safeguarding assets for a company. Certain cities have giving Security officers within certain jurisdiction, such as Boston the power to make arrest do to the lack of public officers availability. The major thing that has changed over time for both public and private security and public servants is the training they must go through before entering the streets or the property they are protecting. The training and education and public officer must get is very hard and they must pass the training and schooling to be hired within the department, it is more in depth compared to what a normal security officer must go through. Security officer now half to go through a lot more training then they use to they must be able to handle people and deal with people on a daily basis. The training for a security officer has really change over the years it’s not only getting a drug test and background check some security companies not do extensive training because of the public views the security. Companies have chosen to educate the security officer because of liability problems that have occurred and exist in the workplace as a result companies have trained professional individuals to work within private security. Normally public officers receive there pay from tax payers and are part of unions that back officers up. With that said most officer are not held accountable for their actions compared to an individual private security  officer. Over the recent years there has been major increases within the hiring of private security officer compared to using local public law officer. It is becoming a huge trend to hire local private security to protect CEOs, political figures, and celebrity figures. They are now using private security companies to be personal protection which are subcontracted out to these individual in need of security. They important people have hired these personal security guards to protected them and also there assets. The small business have also started hiring private security officers to protected them and their assets You use to see Local law enforcement patrolling private property or parking lots but now you have private security taking over those roles. You also have private security such as shopping malls, casinos, and even airports have hired private security officers to protected the business assets. This is not just happening with security guards you also have private investigators who have started taken over the local law enforcement role in investigating murders, rapes, and abductions. Due to not enough funding for public law enforcement, companies like Target have open up their own investigation center that investigate crimes with in there company without having local law enforcement do it for them. Private security companies can now investigate crimes side by side with the police department and also work longer on the case compared to the police department because of funding. Private investigators have been known to build cases against criminals that will support the laws case or it can work the other way and the defendant can hire a private investigator to support their case to prove there innocent. The relationship between the public police and the private security are becoming more positive in a really good way. People are noticing that there is not enough public police officer to protected all of the community so to  let private security take over some of the roles it helps public police do jobs they may actual need to do. Companies have noticed that it is better to have extra eyes watching over there companies. Criminals will also noticed that private security officer and police officer are now working together and are not conflicting with each other and will it will prevent criminals from doing criminal behavior. The only down side for police is the concern that private security officer will take over for local law enforcement and they will be pushed out which puts a negative impact on private security officer to a point where local police officers are upset because they have dedicated their lives to their training and sole propose of becoming a police officer. There is a conflict between public and private space, which in fact can influence the relationship between private security and public police. Police at any time can detain someone that is a threat to someone or can even question them after being apprehend. Private security on the other hand cannot employ any police powers and may only detain and arrest as a citizen. seussical essays The second play I saw for our assignment was Seussical The Musical. This was my second experience seeing a play and I enjoyed it, but not as much as I did with Bat Boy The Musical. I saw the production on Wednesday July 21st at the Workshop Theatre downtown. It was the theatres second week for performing the play. I believe the Workshop Theatre had more seats than the Trustus Threatre, but on the night we saw the performance, the crowd was not as large as it was when I went and saw Bat Boy at Trustus. Seussical the Musical follows the adventures of Horton, an elephant who one day hears voices coming from a speck of dust. He soon discovers that inside this tiny speck exists the smallest planet in the universe. And upon this tiny planet, lived creatures known as Whos. Although other characters disagreed with what Horton believed about the Whos, Horton knew they still existed. Jojo, a child in the city of Who, follows Horton's journey as he is picked on for the unpopular act of thinking. Jojo and Horton attempt to do what they think is right, and together they sing the duet "Alone in the Universe." Horton is sold to the Circus McGurkus and eventually goes on trial before Judge Yertle the Turtle before the other animals of the jungle come to recognize, as Horton has always known, that "a person's a person, no matter how small." During the play, the Cat in the Hat is in and out of scenes, narrating, playing numerous characters, and helping young audience members understand what is go ing on. I think the theme of this play is empowerment. The play focuses on the concept of trusting your instincts, and your abilities and not being afraid to dream, just as Horton did. I chose to go see this play because as a child my parents read Dr. Seuss books to me and this was a way to remember those times and to relate to something I had been exposed to.
